Multi-Agent Systems (MAS) with LLM-based agents face significant challenges in coordination, task alignment, and verification, leading to high failure rates. Researchers from UC Berkeley and Intesa Sanpaolo analyzed 150 tasks and identified 14 failure modes, forming a comprehensive taxonomy. Structural redesign and deeper research are needed to improve MAS reliability, with strategies including enhanced verification mechanisms, standard communication protocols, and reinforcement learning.
