OSHA fined SpaceX $115,850 for seven serious violations after a crane collapse at its Starbase facility in June 2025. The investigation found SpaceX failed to properly inspect a recently repaired hydraulic crane before use, didn't perform required monthly inspections, and had an operator with an expired certification. The incident occurred while cleaning debris from a Starship explosion.
