Microsoft issued an out-of-band update to fix a restart loop caused by its April 2026 Windows Server security update (KB5082063). The original update triggered LSASS crashes on domain controllers in multi-domain forests using Privileged Access Management (PAM), causing repeated restarts and potentially rendering entire domains unavailable. The issue affected Windows Server 2016 through 2025. A separate known issue with the April update may also force some enterprise devices with non-standard BitLocker Group Policy configurations to enter a recovery key on restart. This is the latest in a string of out-of-band patches Microsoft has had to issue, raising questions about its update quality control.
