Great article! I’ll just quickly share how we handled it in my company.
We are usually using what you called <code>Ad-hoc</code> in your text, with exceptions if the merge request is too small. In that case reviewer can just check it out without anyone else.
If it’s something really big, in that case we are going together through the feature / bug fix or whatever it is.
